Failed AgilityTM LP Total Ankle Replacement with Severe >40° Varus Deformity Revision with Prosthesis Conversion and Soft-Tissue Procedures Alone

چکیده
Achieving frontal plane alignment of the ankle joint during primary and revision total ankle replacement is essential for long-term success. This manuscript presents the revision of a failed AgilityTM LP total ankle replacement system with severe > 40° varus ankle deformity that was corrected with explanation and conversion to an INBONETM II total ankle system coupled with soft-tissue release/reconstruction employing percutaneous Achilles tendon lengthening, posterior tibial tendon intramuscular recession lengthening and modified Evans peroneus longus lateral ankle stabilization. Additionally, prophylactic tarsal tunnel release was performed to reduce potential for postoperative nerve compression. At short-term 2.5-years follow-up the patient had a well-balanced and painless foot/ ankle and return of a functional lower limb.

Failed AgilityTM LP Total Ankle Replacement Salvaged with Tibio-Talo- Calcaneal Arthrodesis and Vascularized Pedicled Medial Malleolus and Fibula Onlay Bone Grafts
Controversy exists regarding the most predictable salvage for failed total ankle replacement. Revision total ankle replacement with soft-tissue release/reconstruction or explantation with tibio-talo-calcaneal arthrodesis with bulk structural allograft are the most commonly performed approaches. Tibio-talo-calcaneal arthrodesis with bulk structural allograft has a high complication rate; however...
متن کاملTotal ankle arthroplasty with severe preoperative varus deformity.
Advancements in total ankle arthroplasty (TAA) over the past several decades have led to improved patient outcomes and implant survivorship. Despite these innovations, many implant manufacturers still consider a preoperative coronal plane deformity greater than 10° a relative contraindication to TAA. Without proper intraoperative alignment, these implants may experience abnormal wear and hardwa...
متن کاملTotal ankle replacement in moderate to severe varus deformity of the ankle.
Our study describes the clinical outcome of total ankle replacement (TAR) performed in patients with moderate to severe varus deformity. Between September 2004 and September 2007, 23 ankles with a varus deformity > or = 10 degrees and 22 with neutral alignment received a TAR. Following specific algorithms according to joint congruency, the varus ankles were managed by various additional procedu...

متن کاملCorrection of Tibial Malunion in a Patient with Ipsilateral Total Knee and Ankle Prostheses Using External Ring Fixation and the Ilizarov Method Case Report
Bone, joint, and soft‐tissue deformities of the lower limb have been shown to influence the integrity of ipsilateral joints.[1‐3] For example, increased varus/valgus deformity of an arthritic ankle has been associated with increased arthritis in the ipsilateral knee.[1] One study showed that in patients with varus knee and valgus hindfoot deformities, correction of knee alignment with total kne...
